Hello my name is Katherine Lewis and as the Home Manager I would like to welcome you to the Meadows care home. We are situated in the charming, peaceful village of Johnston, halfway between the town of Haverfordwest and the Maritime Town of Milford Haven.
Meadows is a modern purpose built care setting designed to support the highest standards of 24 hour care to our residents. This friendly and welcoming home offers Nursing and Residential care as well as Respite care which provides a welcomed break for home carers. There is a nurse on duty 24 hours a day.
Our ethos of care delivery is to ensure that those who live with us are supported in an individual person centred way, this is assisted by our Primary Nurse & Key-working system which ensures that individual needs are met and that professional and supportive relationships are developed.
Our Nursing team is not only experienced within dementia and mental health care but can also ensure that physical nursing needs are cared for to a high standard. The home is divided into two units to ensure that assessed needs can be met more effectively and that service users are supported in the most appropriate way.
The Meadows unit provides care for those of our residents who require a greater level of social interaction and stimulation to maintain their current level of independence. This includes reminiscence therapies, individually assessed activities and encouragement to maintain as safe a level of independence as possible.
Within the Willows unit we care for those who require not only mental health care but also a more intense level of physical nursing care to meet their individual needs
To lead our team of care assistants and to ensure that dignity is maintained and standards challenged Helen Thomas works with us as our Social care Practice Leader and Dignity Champion
Activities within the home are led not only by our activities co-ordinator but also by individual staff members who undertake one-to-one activities and more personalised sessions where they promote their key-working skills. Those that live at the Meadows whenever possible are also encouraged to participate in trips out within a fully risk assessed and safe way.
We welcome unannounced visits to the home, so that you can see for yourselves the welcoming atmosphere of our home and assess our performance as it happens each day of the week.
For those who are uncertain about coming to live with us, we also offer trial days, weekends or weeks and can also provide day respite services so that you get to know us before moving into the home.
We aim to provide a home from home experience and provide home cooked nutritious meals based on a traditional menu, with a choice menu to supplement to your liking. We welcome open visiting and accommodate events such as Birthdays and anniversaries when we are more than happy to host a party for you and your friends and family. Although well behaved pets as visitors are more than welcome we are unable to provide accommodation for them, out of consideration for fellow residents.
